Question for truckers or racers with USDOT numbers

It says that in order to get a USDOT NUMBER you must fill out Form MCS-150 and the funny thing is I can't find this form any where even on the Government's own web site Any one know of a direct link to download this form?

All we're trying to do here is haul a race car.

its insane, another way 4 de government 2 get some of ur money, this shuld b a state issue, truckin is gonna get even worse, electronic on board logs 4 drivers is comin july 2014, that means u guys will hav 2 purchase a unit 2 b installed n ur truck to keep ur logs and pre-trip inspections

I found it thanks. My point was that there were all these links to get a dot number and none of them actually went to the form. Yours did work.
